Form R and Tier II Reporting

 
Process Review (SARA Title III, Section 313):

Form R's are required for businesses that meet all three of the following criteria:

  1. SIC Code 20-39 (plus additional categories added in1999)

  2. Has ten or more full time employees

  3. Exceeds the threshold of an aggregately totaled 313 chemical


 
Based on the Hazardous Material Inventory performed, SEIS will complete a
comprehensive review of the manufacturing process(s) and the chemical(s)
used during the calendar year SEIS will determine if the chemical was
manufactured, processed, or otherwise used. This determination will be
compared with the Form R rules to determine if any chemicals were used
​at the facility in excess of the reporting thresholds. 

 

Determine Releases

Once it has been determined that a reporting threshold has been exceeded, SEIS will perform mass balance calculations for each chemical to determine the path and quantity of release.  This release data will be documented to be file with the file copy reports.  Avenues to be quantified include the following:

  1. All stack and point emissions

  2. All fugitive emissions

  3. All waste streams (hazardous waste, trash, etc.)

  4. Discharges to POTW

  5. Drum residue 

  6. Chemical remaining in the final product

  7. Quantity recycled or reused on-site or off-site    


 


Form R Completion and Submission

A five page Form R or two page Form A report will be generated for each chemical. All reports produced by SEIS will be generated, validated and submitted in electronic format as preferred by the EPA. SEIS will generate all required mailing documentation including the certified return receipt mail documents.
